A notice of privacy practices is given to each new patient. This notice explains how health information is disclosed and the legal rights under HIPAA privacy laws. The patient then fills an acknowledgement form and it is placed the patient&#8217;s medical record. In such the medical information is thus considered protected health information and the healthcare provider is a covered entity under the HIPAA Privacy rule. The protected health information must be kept confidential. A violation or breach of confidentiality may result in fines or criminal penalties. But, this doesn’t mean that protected health information can not be disclosed. The law allows disclosure in cases of public health risks or in cases where the patient is a victim of neglect or abuse. Medical data may also be disclosed in cases of national security or to law enforcement. Though in this instances a court order may be required.</p>
<p>A patient has HIPAA rights. But a medical practice must properly treat the patient and also comply with legitimate requests for medical records. It is not so simple as to keep records confidential. All the medical information held must be secure. One way for businesses to keep the information secure and comply with the privacy rules is to use <a href="http://www.edocscan.com">electronic medical records<a/>. Even though, the employee needs to be properly trained and must log off their workstation when not in use.</p>

This game is super-rare: only one is known to exist. These games were created just for this event and then thrown away. The one in existence recently sold on eBay for just over $20,000.</p>
<p><strong>4. Stadium Events (NES)</strong></p>
<p>Because only 200 copies of this game were ever created, released at a handful of stores around the world, it is considered one of the rarest and most valuable video games in the world. Even though there are far fewer copies of Nintendo Campus Challenge 1991, Stadium Events is more playable and isn&#8217;t time-limited, so it is a more valuable video game. A copy of the Bandai version of Stadium Events, brand new in its package, sold at auction for just over $41,000.</p>

<p>This rare sequel to popular Atari title Atlantis was only mailed to the world&#8217;s best Atlantis players. To get your copy of Atlantis II, you had to take a picture of your Atlantis high score and mail it to Atari. If your score was high enough, you got a copy of Atlantis II in the mail. Basically, Atlantis II is just a slightly harder version of Atlantis. New boxed copies of Atlantis II are so rare, they&#8217;re worth as much as $50,000.</p>

Some will probably get the big screen treatment in time, while others will continue to fade deeper into obscurity. Still, a person has the right to fantasize, and these are the film adaptations I would select if I suddenly found myself in charge of a Hollywood studio.</p>
<p><strong>TV Series Begging for Movie Adaptations</strong></p>
<ul>
<li><em><strong>Kolchak: The Night Stalker</strong></em> &#8211; Darren McGavin originally played rumpled reporter Carl Kolchak in the series and a couple of TV movies, and I loved watching him report on occurrences of the supernatural. My pick for the new Carl Kolchak would be Paul Giamatti.</li>
<li><em><strong>The Tick</strong></em> &#8211; Something of a cult classic, this short-lived comedy series starred Patrick Warburton as a superhero who dressed in a blue bodysuit and called himself “The Tick.” Obviously a few bricks short of a load, he fought crime with the help of his neurotic sidekick, Arthur.</li>
<li><em><strong>Buffy the Vampire Slayer</strong></em> &#8211; The Joss Whedon series ran for years and even created a spin-off, so why not bring it to the big screen? After all, it’s not like most of the cast members are doing much these days. I am aware that the whole franchise got started with a motion picture, but I don’t put it anywhere in the same league as the TV show.</li>
<li><em><strong>Supernatural</strong></em> &#8211; The Winchester brothers fight the forces of darkness while roaring across the United States in their 1967 Impala. They’ve also beaten Satan, killed a monster that looked like Paris Hilton, and romanced a whole host of beautiful women (some of them even human).</li>
<li><em><strong>The Shield</strong></em> &#8211; Michael Chiklis was excellent as corrupt cop Vic Mackey, but the supporting cast was always just as fascinating (my fave was Dutch). The long-running series was given an appropriate sendoff, but what self-respecting fan of the series wouldn’t like to see it return?</li>
<li><em><strong>Arrested Development</strong></em> &#8211; This inventive comedy ran for three seasons on television, but it never really found a wide audience. It did, however, make stars out of Jason Bateman and Will Arnett, and I will constantly thank producer Ron Howard for that. While the movie would likely fall short of the series, I’d still like to see the Bluths in action once again.</li>
<li><em><strong>Game of Thrones</strong></em> &#8211; The HBO series based on the works of George R.R. Martin is only one season into its run, and I’m already anticipating a big screen movie. It’s just as well that it never happens, as a feature film would never include all the sex and violence that HBO rolls out in each episode.</li>
<li><em><strong>Profit</strong></em> &#8211; Adrian Pasdar played a ruthless corporate executive who ended every night curled up inside a cardboard box in his lavish apartment. A bizarre series, and one that could be brought to the big screen without most even knowing that the idea came from somewhere else.</li>
</ul>
<p>The odds of these movies getting made are about as likely as <a href="http://www.cultclassics.org/best-spaghetti-western-movies/">spaghetti western movies</a> making a comeback, but hope springs eternal. While most would probably wind up as <a href="http://www.cultclassics.org/">cult classic motion pictures</a>, anyway, I’m going to keep my fingers crossed.</p>

			<content:encoded><![CDATA[<p><strong>Drafting in Fantasy Sports</strong></p>
<p>From my observations of drafts for the 2011-2012 season, here are a few <a title="Fantasy Football Tips - Fantasyomatic" href="http://www.fantasyomatic.com/" target="_blank"><strong>fantasy football tips</strong></a>. When I first got into fantasy sports, I read advice and suggestions from everyone and everywhere. Some of it was actually good advice.</p>
<p>Just as then, some  of my fantasy football tips are obvious, but they bear repeating. Others may not be so obvious.</p>
<p><strong>Having a Low 3rd Round Isn&#8217;t Unfair</strong></p>
<p>I&#8217;ve seen drafts get thin in the bottom half of the 3rd round before. Sure, good players were drafted later, but finding those players became harder. You didn&#8217;t have no-brainers like you did in the first half of the third round, where players seemed more like 2nd rounders.</p>
<p>This year, you&#8217;ll find a lot of good selections still there late in the 3rd round. LaGarrette Blount and Jahvid Best were still available late in that round, while receivers like Miles Austin, Vincent Jackson, and Dwayne Bowe were still out there.</p>
<p><strong>Running Back Options Abound</strong></p>
<p>The top of fantasy football drafts have fewer good options at running back in 2011-12, but the number of running backs with a chance to contribute has grown in recent years. Don&#8217;t forget that in the 7th, 8th, and 9th rounds.</p>
<p><strong>Tight Ends Are Deep</strong></p>
<p>Don&#8217;t draft Antonio Gates in the 4th round, because the tight end position is deeper than ever before. You can find solid tight end performers in lower rounds, so add that second receiver or a quarterback before you draft a tight end. With Antonio Gates, Dallas Clark, Jermichael Finley, and Owen Daniels all injury risks, you&#8217;re probably better off waiting into the 5th round or 6th round and drafting Jason Witten or Vernon Davis.</p>
<p>Vernon Davis especially falls in <a title="Fantasy Football Advice, Suggestions" href="http://blogs.rotoworld.com/Fantasy_Football/" target="_blank"><strong>fantasy football drafts</strong></a>. Owners seem to penalize him for the lack of a good quarterback, but he&#8217;s produced without a good quarterback for years now. In fact, Vernon Davis is quarterback-proof.</p>
<p><strong>Don&#8217;t Draft Quarterbacks High</strong></p>
<p>When I say &#8220;Don&#8217;t draft quarterbacks high&#8221;, I mean don&#8217;t draft them really high. I&#8217;m in a league with a group of players who keep track of the news and don&#8217;t have stupid picks, but the commissioner&#8217;s brother is always asked back. This guy is an absolutely awful player. You know the type: the one who in the middle rounds starts to draft a list of players who were already drafted 3-5 rounds before.</p>
<p>So one of my buddies had the 2nd pick in the draft and wanted to trade down in the 1st. This friend has Adrian Peterson in a keeper league and would rather have a different runner in this league, so he was willing to give up prime real estate for later considerations. So the commissioner&#8217;s brother, when asked about this, says, &#8220;No. That won&#8217;t work with my strategy.&#8221; Sure enough, at the 6th pick in the draft, he selects Michael Vick. At least he didn&#8217;t trade up to do that.</p>
<p>Some people never learn. <p><strong>What is the mystery behind the placebo effect?</strong></p>
<p>While the placebos do not really promise any cure, many researches are wondering why it has an effect in the person taking it. 1 out of 3 patients claim a change of symptoms regarding their disease after taking placebo pills, treatment and injection.  Placebo pills are known to reduce the symptoms of illnesses. The effects would last depending on the belief of the individual who took it. At times, they can also cause side effects called nocebo effect. If the person who has taken it believes that the pill and treatment can cure them and has faith in the process, then a person can start feeling better even without taking real medicine. What is important is the person should not know that it is a placebo pill or treatment. It would work just as long as patients believe that they would be healed because of the treatment that has been provided.</p>
<p><strong>Where are placebos used?</strong></p>
<p>They are used in clinical trials, research studies regarding new drugs and treatments with volunteers. Before the treatment is even tested in the lab, they undergo placebo effect testing.  The next step is to try this with animals. If the medicine or the treatment gives promising results, they will be subjected to other clinical trials using patients. Patients would then be asked if they felt that the treatment is effective. They would be asked if the treatment that was given is better than other treatments. They would also be questioned for side effects that they felt, and for other benefits that the treatment has given.</p>
<p>Placebos are being used in order to make sure that the new treatment is the one giving the effect and not other factors. The placebo pills, treatments and methods would resemble the actual treatment. The doctors and the patients in the test would not know where the real cure is in order to remove the biases for the benefit of the research.</p>

			<content:encoded><![CDATA[<p>If anyone is capable of reviewing the worst Internet games of the year, it&#8217;s me. I spend so much time playing online games at work and at home that I&#8217;ve had to start using the <a href="http://www.alabamabroadband.org/lockfox/">Lockfox privacy browser</a> so that no one looks at my Internet history and catches me in my addiction.</p>
<p>Lists of the <a href="http://www.internetspecials.org/internet/games/">best Internet games</a> of the year are all over the place. If you want to know the best Internet games of 2011, look elsewhere. Here&#8217;s a look at my least favorite&#8211;the absolute worst Internet games of 2011.</p>
<p>I&#8217;m so sick of Crush the Castle in all its various incarnations I could actually puke. The first version was a cute distraction, but how many times can you fling rocks and bombs at increasingly thicker castle walls before you get bored and (God forbid) start doing some actual work? It seems like there&#8217;s a new version of Crush the Castle or some other siege game every time I get online.</p>
<p>Another big disappointment this year was the much talked about Battlestar Galactica MMORPG. As a huge fan of the series reboot, I was expecting a combination strategy and FPS sort of game, but what we got instead was a heavily watered-down version of the game featuring little to no strategy and lots of shoot-em-up action. Even on my brand-new laptop, the game was clunky and slow. I give it big props for the graphics&#8211;some of the characters look remarkably like the actors from the TV series&#8211;but there&#8217;s nothing else good about this MMORPG.</p>
<p>My final complaint about 2011&#8242;s slate of Internet games is that online gaming seems to have gone the way of Hollywood, with nothing but sequels, prequels, and expansion packs. Where are all the new games? I don&#8217;t want to play The Last Stand 2 or Elona Shooter 3, I want to play new games with new features and a new story line. Indie game producers have created some new interesting ways to game online, but who really wants another iteration of Facebook&#8217;s same-old same-old multi-user sandbox games?</p>
